JEE Main 2021 from 23 Feb – Download Admit Card, Undertaking at jeemain.nta.nic.in; Know How To Fill Self Declaration Form

It is mandatory to take the correctly filled in self declaration form, aka undertaking, to the exam center on the day of exam.

JEE Main 2021 February examination is just around the corner. The exam is due to be held February 23 – 26, 2021. All candidates appearing in the exam must download their admit card / hall ticket from jeemain.nta.nic.in now. The said document contains exam date, time, and center.

Talking about the admit card, on the exam day you also have to carry a print out of the JEE Main 2021 undertaking / self declaration form as well. This form is available at the bottom of the admit card and is mandatory to carry on the exam day. However, it should be filled in the correct format.

How To Fill JEE Main 2021 Undertaking / Self Declaration Form?

JEE Main undertaking / self declaration form is a mandatory form which all the aspirants of JEE Main 2021 need to fill. In is in regard to COVID-19 and states that an aspirant has read all instructions and guidelines pertaining to COVID-19. The JEE Main 2021 admit card has 3 pages. One of those pages is the JEE Main 2021 Undertaking / Self Declaration Form. Here is how to fill it:

There are basically 8 important parts of JEE Main undertaking / self declaration form and these are as follows:

Symptoms: Check one or more boxes out of the available option here, as applicable. If no such symptoms are faced by you in the past 14 days leave this out.

Other symptoms, if any: Also, if you are experiencing any other symptoms that is not mentioned here, describe it in the space given. If no such symptoms are faced by you in the past 14 days leave this out.

Contact with COVID-19 cases: Check this box if you have been in close contact with a confirmed case of the COVID-19. (‘Close contact’ means being at less than one meter for more than 15 minutes.)

Quarantine details: Check this box if you have not been in close contact with a person suffering from COVID-19 and are NOT under mandatory quarantine.

Travel History: If you have a travel history in the last 14 days then you have to mention the name of the places and date of travel in the table provided.

Photo: Here paste your Photo (Same as uploaded on the application form to be pasted before reaching the center).

Thumb impression: Then here you have to put the left-hand thumb impression(To be put before reaching the center).

Signature: This is NOT to be filled before entering the exam hall. This space is to be filled in presence of the invigilators on the day of Examination.
